The challenge
ClearPath's urgent care network was triaging patients manually across 22 clinics, producing wildly inconsistent wait times and clinical prioritisation errors.
ClearPath Health operates 22 urgent care clinics across the Southeast US. Inconsistent triage practices across sites — driven by varying staff experience, shift-specific pressures, and no standardised protocol — meant patients presenting with similar symptoms were being assessed and waited very differently depending on which clinic they walked into. Clinical leadership wanted a system that could standardise triage quality, reduce unnecessary waits, and flag high-acuity patients immediately.
Our approach
Clinical protocol digitalisation
We began by working with ClearPath's medical directors to formalise their triage protocols into a structured decision framework — identifying the clinical variables, symptom clusters, and risk factors that should drive prioritisation at each acuity level. This became the foundation of the model's training target.
NLP-powered intake processing
A natural language processing layer extracts structured clinical signals from freeform intake notes — the way clinicians actually document at speed. The model maps these to the structured protocol in real time, reducing the burden on intake staff while improving data quality downstream.
Care pathway routing
Beyond triage scoring, the system recommends the appropriate care pathway for each patient — fast track, standard care, or immediate escalation — and routes the case to the right clinical queue. High-acuity flags trigger an alert to the duty clinician directly.
Multi-site rollout & calibration
The system was deployed site by site over 16 weeks, with a two-week shadow period at each clinic where outputs were reviewed by clinical leads before going live. Site-specific calibration addressed population differences between urban and suburban patient profiles.
